Our agency is a vast resource for persons in need of professional care in the home. Forum Health Care provides a wide array of services in the patient’s home to assist with recovery and health care.
As a home health agency, Forum Health Care provides the following services:
- Skilled Nursing (SN)
Registered Nurse (RN) will make the first visit to patient’s home. RN will work closely with patient’s primary care physician to develop an appropriate care plan. RN will provide skilled nursing services such as drawing blood for lab tests, dressing changes, wound care, injections, and also teaching the patient about medication and treatment. Our multicultural nursing staff consists of High Tech Nurses, Certified Psychiatric Nurses, and wound care specialists
- Physical Therapy (PT)
A Physical Therapist (PT) will evaluate patient’s condition and set up a home exercise program and plan treatment intended to restore and preserve function. PT will perform gait training, and instruct patient in assistive and exercise programs to relieve muscle and joint pain and restore muscle function
- Occupational Therapy (OT)
Occupational Therapist (OT) will guide and instruct patients in caring for themselves such as activities of daily living to improve independent functioning. OT will provide patient teaching to perform routine functions such as dressing, bathing, caring for a home and living with adaptive equipment
- Licensed Clinical Social Worker (LCSW)
Licensed Clinical Social Worker (LCSW) will provide service to patients with social and emotional concerns, with personal or financial issues while they are recovering from an illness or disability. LCSW will create a plan for continued service after discharge and act as liaisons with other community agencies
- Home Health Aid (HHA)
Home Health Aid (HHA), the Certified Nursing Assistant, will provide assistance with personal care with activities for daily living (bath, shampoo, mouth, foot, skin care, motion exercises) until the patient and/or caregiver are independent and able to independently perform this care.
- Speech Therapy (ST)
Speech Therapist (ST) will evaluate and assist people with speech, communication, and swallowing disorders, helping individuals and families on speech problems and on how to cope with the stress associated with these problems. Therapists also work with families on treatment techniques to use at home and on how to modify behavior that impedes communication
- Psychiatric Nursing (RN)
A qualified Psychiatric Nurse (RN) will do the initial assessment and ongoing reassessments for patients with psychiatric needs who may be under the care of a Medical Physician or Psychiatrist. Those assessments are addressed to the emotional and behavioral aspects of functioning, mental status examination that is appropriate to the patient’s age, and use and side effects of psychoactive drugs
